appealing the refeers decision

i resigned from my job of 12yrs during my employ i incurred health problems,carpal tunnel arthritis,migraines,i suffered,and my company knew-the workload by which i carried was huge-i was advised by my doctors to move to warmer climate-fortunately my daughter resides in az,i had the phone hearing an was denied, now i have to write to board of appeal–i had good cause for leaving that job–how do i prove to them,
