i was termianted becasue i couldnt give doctor information to my benefit dept by the deadline. I told the lady i never received the paper, she said the deadline past and i would be terminated.
My guess, is that it will ultimately depend on whether the employer can “verify” that you should have received that paperwork to be filled out by your doctor.
